# Why isn't the hip the same as the knee?

## Why can't knee advice answer every hip question?

## Could the soreness be outside the joint?

I'd show the clinician the worst spot with one finger.

Expect the clinician to press around your hip and move your leg. You may also be asked to walk a short distance. These checks help decide whether the joint or the tissue around it is sore.

## Why don't knee results carry over?

A knee study may include people with different wear and different daily limits. Treatment is also placed differently because the hip lies deeper. Good knee results can give a clinician useful background. They still don't prove that your hip will respond the same way.

Ask which results came from hips.

You can say, "Was this treatment studied in hips or only in knees?" Then ask what the people in that study could do afterward. Soreness scores matter, but so do walking, sleep and getting out of a chair.

## Are hip replacement alternatives the same as putting surgery off?

No. Hip replacement alternatives are choices considered before replacing the joint. They can include exercise, using a cane, changing hard activities or trying non-surgical care. Trying one doesn't rule out surgery later. An X-ray alone doesn't set the date. Your sleep, walking and ability to manage chores matter too.

There isn't one timetable for every worn hip.

Ask what would make surgery useful later. Also ask whether waiting could make your daily life harder. You deserve a clear answer about trying another choice first.

## What if the hip stays sore after home care?

The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

The 2026 Cochrane update of exercise for hip osteoarthritis (18 trials, 1368 participants) found that against attention control or placebo, exercise may have little to no effect on pain (MD -6.31 points on 0-100, 95% CI -12.98 to 0.35, low certainty) and may improve physical function only slightly; against no treatment or usual care it probably reduces pain slightly (MD -7.19, 95% CI -10.70 to -3.68, moderate certainty) but the review states these improvements are unlikely to be clinically meaningful. This is a weaker result than the equivalent knee evidence and it must not be overstated.
